BIOMARKERS & SUPPLEMENTS for mitochondrial health optimization

Mitochondria are truly the “powerhouse” of the cell and there is active research (and debate) as to which laboratory biomarkers are clinical correlates of mitochondrial health. 

Pregnenolone is a biomarker that we trend and “optimize” for our patients. Is Pregnenolone a marker or mitochondrial health? It certainly is a marker of aging and stress, it’s also lowered in patients who take certain medications (statins, antidepressants, etc). 

Pregnenolone is not a direct biomarker of mitochondrial health but can serve as an indirect indicator due to its synthesis in mitochondria and its role in supporting mitochondrial function. Low pregnenolone levels may suggest impaired mitochondrial activity, particularly in steroidogenic tissues like the adrenals, gonads, and brain, as its production depends on healthy mitochondrial enzymes (e.g., cytochrome P450scc). However, pregnenolone levels are influenced by many factors—stress, hormonal imbalances, aging, and diet—so they lack specificity as a standalone marker of mitochondrial health.

Coenzyme Q10 (CoQ10) is an essential mitochondrial cofactor, too. Low levels of CoQ suggest reduced mitochondrial electron transport chain efficiency, as CoQ10 is critical for ATP (cellular energy) production. CoQ is often depleted when taking commonly prescribed medications, especially statins.

New Study Update on Semaglutide and Eye Health!

Semaglutide (Ozempic, Wegovy) has transformed diabetes and obesity care—but concerns were raised about a potential risk of NAION (nonarteritic anterior ischemic optic neuropathy), a "stroke of the optic nerve."

What this new study found:

297,000+ patients from 21 countries1:1 matched for age, sex, BMI, HbA1c, comorbidities, and medication useNO significant increase in NAION risk with semaglutide compared to other diabetes/weight-loss meds Consistent across T2DM-only, Obesity-only, and T2DM+Obesity groupsResults held steady at 1, 2, and 3 years of follow-up

Key takeaway:

Avoiding semaglutide solely over NAION fears isn't supported by real-world data. Its cardiometabolic benefits (glucose control, stroke prevention, weight loss) still far outweigh potential risks.
